It is not unusual to upgrade someone with a broken leg or arm because of lack of maneuverability. This is not much different.
Actually yes it is unusual to upgrade someone with a broken leg or arm. They might shift their seat in the cabin they are booked in order to best accommodate them, but they aren't going to upgrade someone just because they show up in a cast (or pregnant).
