Flying Pregnant [op-up?]

I am going to visit my sister in Italy at the end of November (we booked our tickets in May). Unexpectedly, but happily, I found out I am pregnant and will be 22 and 23 weeks, respectively, when I fly to Europe and back to the States. My mom and teenage brother are also flying and booked their own tickets on the same flights. Barring any complications in the next two months, my doctor has cleared me to take the trip. What are the chances of being upgraded to a larger seat or a better location? I am 5 feet tall and this is my second child so I will be showing a lot. I know I should bring a doctors note that OK's me to fly. Is there anything else I can do to increase my chance of an upgrade?
