That has happened to me before more than once. The agent on the phone kept saying she could not see the upgrade I had to tell her to "clean" the booking and voilą the upgrade was there.

I agree with canadiancow that this usually happens between time of request and check-in.

So now I always phone and ask to have the booking cleaned, just to be on the safe side
