Was the passenger denied a visa or just didn't apply? If the former the refund fee might be waived with a call to the company. I've had revenue tickets in the past where that was explicitly in the fare rules and getting a carrier to be cooperative in such a scenario is not unheard of. But it is also not policy AFAIK so you're at the mercy of the agents.
