I'm a big fan of the Access Express, and have been taking it since it started. It's great value, and very competitive in terms of trip time -- certainly for anything on its path (Conrad, IC Strings/TB), but also, you can easily beat the limousine bus if your destination is e.g., GH, Andaz, or Westin. One negative I have noticed is that it's getting much more packed than it used to in the beginning. It wasn't uncommon to have less than a dozen riders in a compartment, and now by the end of the journey, usually all the seating is taken and there are some standees.
In addition to where your hotel is, I think it also depends on the time of the day, time of the year, how much stuff you have in tow. Stayed at Takanawa Prince recently, which is ~7min walk to the station. With that killer heat/humidity, to me the Limo Bus was a no-brainer. Even if it was a 3min walk to the station, I still would've taken the Limo Bus. Personally I'd gladly pay the extra 1500y to not have to break a sweat prior to a long TPAC flight. The Limo Bus arrived at NRT >20min early, which is fairly common.
