Correct.

Although once, I was upgraded by Maradori - confirmed. Afterwards, I had to change my flight. The GA undid the upgrade, changed my flight, and reprocessed the upgrade without me having to bug Maradori. Not sure if this was SOP..

But otherwise, your understanding is correct.
