OP I think it would help if you are able to find the booking class and as much detail as you can for the fare you paid, so the forum experts can help determine what your expectation is vis-à-vis a refund.
In terms of the separate issues, I would be calling up to get the cancellation notice for your travel insurance. Many people find their emails are skimmed rather than read and consequently the main point of the email is often not dealt with.
As far as the actual refund itself, I'm not sure how much the forum can help with the timings on that. There is a slow grinding process that does eventually work but it can take several weeks. Of course that is not ideal and frankly fairly poor, but it's happening to everyone so try not to take it to heart.
I would definitely be chasing the cancellation notice with some vigour, and let the wheels of time slowly tick away for the refund request. Best of luck.
