I can fly out of LAX as PHX-LAX positioning flights are always cheap with money or miles. ^^

It is definitely harder in a hub city like a PHX for AA or SLC for DL which are less of a major international gateway and more of a domestic hub i'll give you that.

Not really. I just got 50k Miles from AA for getting their CC and spending $3k. And I don't mind flying them as they have great nonstop PHX-PDX service and I fly there regularly to visit friends and family. And I have never had a bad experience on their (US Air) flights.
