I recently got upgraded two car classes at BOS. The receipt the exit-booth attendant gave me upon exiting the lot said approximately this: "Rate -- Economy/Car Given -- Intermediate." Or something like that. It was quite clear and was billed correctly.

I got a Sentra, which was pretty good for $16/day plus taxes & fees, i.e., about $65 for the weekend given that this was BOS.
