I haven't had precisely that happen, but it appears that it's all too easy for a phone agent to totally screw up a reservation when trying to do something. (I had SFO-JFK changed to something involving Mexico when I tried to switch to SFO-LAX-JFK due to delays on my flight. Fortunately, the next agent could fix it.)
