I guess I was just lucky. I threw up several times during the last 2 or 3 hours of a United flight from Sao Paulo to Houston a couple of weeks ago. I was so sick they had to order a wheel chair to take me to my connecting flight. I told the flight attendants it was due to a migraine (which was the truth), but they had no reason to trust my diagnosis. No doctor or paramedic was called. They just carted me off in the wheelchair through immigration and customs to my next, also United, flight.
