If the name on the ticket is exactly the same as the M&B account, then you don't need (and shouldn't) fill in the box for the "surname as on ticket" and "name on ticket".
In regards to flight number, You should just write the flight number, not prefix (e.g. TK for Turkish) just write the numbers. In my experience, i have always written the flight number without the zeros, for example 1 and not 0001, and i have never had a problem, although it has been reported somewhere on this A3 forum that an A3 agent once told someone that the reason his miles had not credited was because he didn't enter the flight number in 4 digit format (but that was not the only reason offered).
