If your "personal and medical" reasons involve being near a lavatory, it's probably more important to be in an aisle seat in one of the rear rows than in the very last row.

Planes the size of a 777 usually also have lavs somewhere else further forward in the economy cabin. You might also consider reserving a seat near that one.
