Generally they have normal recline. I would personally not rely on the 'SeatGuru' type sites, which are on the whole out of date and often not based on actual experience.

Not sure what 'personal and medical' reason you would have for wanting to reserve the last row in economy. Note that some airlines will block the last row(s) for group bookings, unaccompanied minors, etc so you would not be able to reserve them anyway.
