Just received my $650 statement credit, less the $95 annual fee when my statement closed today. I made the phone purchase on 5/8.
e: Also, go to Account Management and select Credit Balance Refund to get a check for the negative balance.
e2: Cancelled AT&T service on the 16th or 17th day.
