Some airlines (IIRC primarily UA but maybe it was AA) have been hit with substantial fines from DOT for the slow processing of refunds. These may have been mandatory refunds, such as from cancelled flights or the 24 hour refund (which would apply to UA but not AA, which has chosen the 24 hour hold).
