No, you can't switch the ticket from one name to another. You would book a ticket under her name and leave off the RR number. To be certain to save a spot for yourself, book a fully refundable ticket for you. When you're ready to divulge the info, book the companion ticket, etc, simply call up/add the RR number to her flight, book the companion ticket, then cancel your refundable ticket. If the flight had sold out by these last steps, you could then call up customer service and convert *your* refundable ticket to a companion pass.
