You won't get the cash back on the order using the promo code the first time. After you get the free shipping attached to your amex account, you will get cash back.

Read the posts mentioned in the post above yours for the instructions on how to do it. Worst case you pay 3.95 for one card and lose cashback on two $25 orders.
