It's my understanding that is considered a serious operations failure (a competent regulator should consider this to be a serious problem). If US has any systemic deficiencies here, that's a Problem. Not to deny what you are saying, but to emphasise that's a serious problem with significant adverse consequences.

You have only to imagine the headlines if US has an accident on one of its aircraft, "Mystery man on doomed US flight - did he bring it down?" ... "US Airways says it cannot account for all the passengers on board the aircraft that crashed last Tuesday in mysterious circumstances... Operations Director Luck Less admitted reports on Internet websites that US Airways frequently marks passengers as no-show when they have boarded 'may be true in some cases'" etc etc etc. Fill in your own hyperbole

In OP's scenario, the headcount taken by the FA's ought to have exceeded the scanned count by one.
Does US do a headcount of passengers on board when they have boarded (not all airlines do)?
