IIRC there were some FB posts not long ago with a link to download a form.

IB Plus form

I guess the form together with ID, either faxed or emailed, should be enough to unblock the account.

Emails sent to IB service centres have to include the IB number in the subject line (only the 8 digits), otherwise delivery fails.
