+1. While I agree that the form is quite aggressive - in contents (very generic), form (we may have no choice) and deadline, now is probably a good time to figure out for yourself where you stand on the substance.

The email clearly focuses on the flights tracked to your account, ie accrual, so if you have any reason to think that for instance, a flight has been tracked that someone else has flown or any other irregularity that you can think of, it is probably best to come out clean with them.
