Any idea what triggered their suspicion? I would be concerned at their approach - like a policeman saying out of the blue "sign here to swear you've done nothing wrong, you have to sign, emails and verbal confirmation won't do". It would instantly raise my suspicions as to what dirt they had on me.

+1. While I agree that the form is quite aggressive - in contents (very generic), form (we may have no choice) and deadline, now is probably a good time to figure out for yourself where you stand on the substance.
The email clearly focuses on the flights tracked to your account, ie accrual, so if you have any reason to think that for instance, a flight has been tracked that someone else has flown or any other irregularity that you can think of, it is probably best to come out clean with them.
