I guess the main difference with this and most instance where people do this, is that it is a continuing flight with a single boarding pass to DXB. If you were on a later flight to DXB it would have been simpler.
Nevertheless, I'm in the camp where you communicate that you cannot take the last leg at the time and do not call before hand.
If communicated at check in, the check in agent may send the person to the ticketing desk to fix it
If doing it as a one off , then just not reboarding will not hold anyone up. If the passenger fails to board then the flight will just depart without the passenger
If going to inform someone, be prepared to pay accordingly
The main concern to me would be to verify whether passengers can disembark in MXP if continuing on
