Not sure you did anything wrong but that doesn't mean the system isn't messed up. I don't know how you got to the schedule appointment stage without the conditional approval letter being posted to your record. Unless you accidently deleted the notification letter it sounds like a system mistake or glitch and I have no idea how to fix it. I do know that for our interviews the first things asked for were the conditional approval letter and our passports.
