WSJ reports they were quoting from the FAA letter itself. The quotes from that letter include the following:
In this case, according to the FAA letter, voluntary reports of 12 apparent violations in 13 months stemming “from the same or similar set of circumstances” indicate a “systemic” hazard, requiring United to conduct “a complete review of your processes associated with crew member qualification” and develop an “action plan…to mitigate this hazard.”
You can be sure this particular story (and these quotes) were carefully vetted for accuracy prior to publication. The risk of a defamation suit is otherwise far too high.

I appreciate the input in this thread from the UA pilots and would fly very comfortably indeed on their aircraft.

But I'm having a hard time with some of the comments here which pretend the FAA did not express the concerns they very obviously did.
