Only there's no where on the form to enter the class of service. What gives? Or is that a pointless question?
There should be on the far right hand side. You would enter your fare class as noted on your ticket. I entered E as noted on my ticket and it still rejected though. Ended up just calling it in...and paid $25 for doing so.
