I don't recall this being specifically addressed. While I hope you get those benefits, I'm curious as to how it would work. Would someone with 5M US lifetime miles (granted, there probably aren't many such people) get 35K miles and 16 SWUs?

I believe a reasonable case can be made for a benefit based on the last milestone, which in your case would be the 4 SWUs for passing 2M. I wouldn't count on the 35K miles for passing 1M.
