This happens to me a lot (I am in the US). Generally it's when I have done an award search looking for one date, then go to edit the form and put in another date; the second try usually results in DD/MM format rather than the MM/DD format used in search #1. To avoid this problem, for subsequent searches I hit the "start again" button, or reload the form from the very beginning.
