Uber Black because you get a professional driver who knows alternate routes if there is an unforseen delay, e.g., traffic accident / construction. I always do this on any time-sensitive trip. The extra cost is not that significant versus the cost of delay.

Allow 45 mins from Midtown to JFK at that time of night. Presuming pre-check + no bags to check and OLCI should only be 15 mins from curbside to gate at JFK.

If you want to squeeze a few extra minutes at your dinner, book a car through Elite or Dial7. You then don't need to build in extra time to wait for your car.
