I have been on several UA flights between Australia and USA and vice versa, sitting on upper deck of 747s, in the past 5+ years and I can say for sure that when one of the pilots gone to the toilet, no FA entered the cockpit. All they did was blocking the access to the lavatories and the cockpit.

Have also been on TG 747 flights sitting on upper deck and as far as I can recall, no FA entered cockpit during toilet break of one of the pilots. No such experience on LH, SQ, LX but I am comfortable saying that such policy is familiar only and only to (some) overzealous US based carriers...

Been on few QF and VA flights in Australia where I would be comfortable to again say that when one of the pilots goes to the toilet, no FA replaces him/her (yes, there are even few captain ranked females in Australia) in the cockpit...
