Has anyone seen this before:

Due to the special nature of the rate booked, a rate authorization form obtained by the Starwood Associate must be presented to the hotel upon check-in. Without this form, the hotel is not obligated to honor your reserved rate and you will be charged a higher one.

i was booking a reservation, via the SPG iphone app, when a special rate that I have never heard of before came up. It's about 70% cheaper than the currently lowest price. so I booked it, and in my email confirmation, the above rate language showed up as part of the email.

1) How do you go about getting a rate authorization form? Do I just call SPG customer service?
2) Is this rate eligible for earning starwood points (and stay/night credit?) If it's a rate that I obtained through SPG (the app, but should be the same thing), then I would believe it should be eligible. But since it's such a lower rate, 70% lower than the current lowest, non-refundable rate, I just want to make sure.
