Actually a small airport is great. Arrive just after the scheduled departure time of a flight on say a Saturday morning. If the next flight is a few hours away, then most likely there will be nobody in line. I've redeemed vouchers that way - was so quick my ride only had to circle the airport once.
IME, it takes much, much longer to process two forms of payment than a voucher. I wasn't kidding when I said it took nearly an hour, and that was walking right up to the desk, with no wait on line.
I first tried to redeem at HPN on a Saturday, when AA generally has two counter agents. I waited until the check-in line dwindled for one flight, and then people started showing up for the next flight. Eventually one of the agents took pity on me and said they very rarely process two-payment tickets because there just isn't enough time.
I gave up and went to a larger airport.
