Just a warning:
I ignored the request for all that info and all of my credit card accounts got a closed status online. Phone rep said they will be closed in 10 days after I called. all purchases declined. I transferred all the points out just to be safe. Now I have to go to the branch and fill out all that paperwork asap so that the accounts get out of that closed limbo status.

Once before I had gotten the forms, gone in to a branch and the rep did not know what to do with them or how to help me with them so I told her "I am here to help you and your request, if you dont know what to do I am throwing them out" . That was years ago and that's why I ignored this request.

After this incidence, I will not be ignoring those forms. I like my chase cards too much.
