You can book tickets on-line at thaiairways.com. Just plug in your cities and when you get to the end of the booking process, it asks you if you want e-tickets, (only for limited international) *edit* SIN-HKT is not one of the international routes. and all domestic routes *edit* i.e: BKK-HKT. The web site has a currency converter so that you can see what you are paying in your own currency. They accept, if memory serves correctly, Visa, Master Card, Diners & Amex. Be sure to have the same card with you along with the e-ticket which is e-mailed to you quite promptly.

You need to set up a profile to book, just as on most other airlines' websites.

*edit* I just booked, but have not yet ticketed the following with Malaysia Airlines (impossible to do on-line) 1-800-552-9294. Hours approximately 0700-1600 PST everyday. They have to contact the rate desk, but costs of a ticket HKT-KUL and KUL-JHB (Johor Bahru) across the causeway from Singapore it was $146.60 in H class. This was done with separate tickets for a stopover in KUL. SIN was approximately $35.00 more. These rates are one-way. You double them for RT.

You can also book on Expedia and Travelocity and pay the ticket fee. Malaysia will send paper tickets from Los Angeles at no additional cost, thus saving $10-$20 per booking. Thai provides e-tickets. You cannot book paper tickets with Thai unless you pick them up in the country of travel origination within 72 hours, so you must e-ticket.

You can get UA miles on Thai and NW miles on Malaysia. *end edit*

Seems quite simple and I will test the system in less than a month.
