As a US citizen, I don't have to deal with ESTA, etc. That said, I would avoid the doctor's note and saying that she's currently unable to work for medical reasons. I think she runs the risk of rubbing the official the wrong way because they might think to themselves, "Oh, she's too sick to work but not too sick to take a vacation?"
Proof of funds, a return ticket, a lease agreement, etc., would all demonstrate her intention to go home at the end of her trip.
