I filled in the form on the plane, but a lot of HFP comments suggested these too often get lost, so also sent an e-mail to Virgin CS.
They responded in around 24 hours, asking me for a picture of my BA Gold card, the booking reference from my last Virgin flight, and it was applied c. 24 hours after that.
All dead simple.
