They cancel the tickets without hesitation. It's clearly written in the CoC and their have been reports of this happening several times.
No IDB. It's the same as having a AAA-BBB ticket and the pax insisting they fly AAA-CCC. DOT would get a simple response from airline that the pax violated the COC and the complaint would go into a memory hole.
