Yeah, I can almost guarantee you won't get any points or vouchers for this. It seems to vary as to what exactly you're entitled to, based on what the SD decides.

The "business card" sized forms vary from 5-15% (economy to business)
Fill in the LONG form - Points or $150 evoucher

My experience was a broken power outlet, AC810 (YYZ-IST), on a J upgrade. I was given the long form, and a few weeks later received the evoucher. IFE worked fine, I just couldn't power my laptop to work on it.
