Based on the OP's program designation of "Qantas," I'm assuming he/she's based in/near Australia. Rentals booked through European/Australian/other overseas sites are often all-inclusive, so I'm guessing that's what he/she's referring to.

As for including the cost of any prepurchased CDW, those are usually only available from third-party OTAs like Orbitz, Expedia, etc., and AFAIK, they are itemized separately from the cost of the reservation. (Note that these are separate insurance products sold by companies like Allianz and TravelGuard--you're not actually prepaying for the CDW with the rental company itself.) So it should be fairly easy to "unbundle" the cost of CDW insurance if you've bought it from a third-party site, but I don't think that's what the OP is referring to.
