If you arrive at HND International, bear in mind there is only one shuttle (2:40pm) within your time frame…or you can always take a free shuttle to T1/T2 where the buses are more frequent.

Alternatively you can do (as mentioned in posts above) Monorail + NEX/Monorail + Yamanote + Skyliner or as suggested by gnaget the Access Express that takes you directly to Narita.

I did this connection (almost exact arrival/departure times!!) and I had time to devour in sushi at NRT
