Apologize for my ignorance, but when you say you sent reimbursement request in by "SM," does that mean "snail mail?" If so, what is that address (I read the wiki, but doesn't have it). I have been approved for the card, but not yet received it . . . if the SM address is in the materials that come with the card, just let me know that, thanks.
