Worth adding another data point here - I read a trip report yesterday of a passenger flying USA-PVG-GUM (UA-UA). Passenger confirmed with airline, and was permitted to board in USA without visa, but was of course stopped by Chinese immigration.

The passenger was eventually allowed to 'enter' for the purposes of check-in and allowed to board their flight to GUM, but they say it was not without some scary moments.
So they are taking the position that any protectorate or territory of a country does not qualify as a third country? Ie for the US; Guam, Saipan, USVI, Puerto Rico, etc?
