An outside possibility is that the card number and pin entry were caught by the security cameras typically over the service desks where MOs can be obtained. In my dealings with Winn-Dixie I have actually seen the continuous camera output. Depending on the placement and resolution of the camera, there is some opportunity for shoulder surfing, a classic card fraud technique.

With all the cell phone cameras out there, it is also easy for a thief behind you to shoulder surf card numbers while you hold the card in line or on the counter. In some cases the default pins are the last four digits of the card, so an image of the card would be enough to commit fraud.

Also watch out for clerks with cell phones at the counter who could take card pictures as well.

So, on 14th I bought 2 x $500 "Celebrate" Visas at Kroger (mygiftcardsite.com).
Cards were sealed in usual covers and untempered. On 14th I partially emptied them by getting $300 MO and leaving it with $200 balance for something else. That was the only time I used it and never showed it to anyone else including money center clerk. Last night both cards had small pending charges from GAP and amazon dated on 16th. One of the card was registered to some weird name but no address (had greeting like Welcome Darnell Smith, but when I clicked details of the card, the address fields were empty).
So how and at which point do you think these cards were compromised?
