Stairs + unexpected turbulence = injury, especially if the passengers are already a little unsteady on their feet (old, injured, small children). I have been on a couple of aircraft that have the loos downstairs (Lufthansa, plus a British carrier I can't recall - could have been a charter?) and whilst I didn't have any issues, I recall thinking that someone really could get hurt if there was an unexpected bump.
