If it is not in Control Panel's Add/Remove programs, and if Ad-Aware did not pick it up (probably because it's a new software and it's not in their databases yet or something), try this: Open up IE, go to Tools -> Options, on the General Tab in the middle of the screen click on Settings and on the settings screen click on View Objects. If you're lucky, it might show up there. You'll see Update Class ( for windows update) and Shockwave plugin probably, and some other ones, but right-click on any of those that have weird/strange names or those that you dont recognize, go to properties and you will see listed the url link where it came from. Now if you dont remember the website for the survey, you could try ruling out others, just by looking at their properties and perhaps recognizing the URL link where they originally came from.

I, for one did not opt to take the survey, just because of that stupid plug-in which i was not familiar with and did not know what it might do to my computer.
If the above steps are unsuccessful, you might want to try System Restore if you're running Windows XP, or perhaps, as a last resort, if you're really concerned about what data might that software *still* collect from you, just back up your important files and reinstall the OS.
