Worst I've ever experienced in North America was YUL to any point east of downtown Montreal. Last time we did it, it was over 2 hours to the Chambly area. Absolutely awful. We quickly figured out what everybody else east of Montreal, American or Canadian, figured out long ago: fly in/out of BTV.

DEN is almost never a problem getting *in* to Denver, although I'm usually arriving towards the end of rush hour. Can somebody be a bit painful going out to DEN to catch a late afternoon flight. You really need to be on I-70 by 3PM at the latest: it always surprises me how early rush hour starts there.

IAD is painful if you're trying to get to downtown DC, but I almost never do that. I used IAD more often when we had an office in Reston: for that it was perfect.

MCI is always easy whether you're going to downtown or to Kansas.

CVG isn't even really *far*. 20 minutes to Cincy...not sure why it's even on this list.

DFW can be a pain if your destination is Dallas: especially if you land on a runway far from your gate and doubly so if you are renting a car. It feels like you cover 10 miles taxiing and another 10 miles on a rental car bus and another 10 miles just to get off airport property! DAL is so much better for anything in the Dallas/Addison/Plano area. DFW can add an hour to your trip even if the traffic isn't that bad.
