Gigafast are very slow to pay out. I got my rebate about 14 weeks after buying the card.

While the card works, it's of a pretty poor standard: It's driver crashes every time I change a PCMCIA card in my machine, it has no Linux drivers, and it doesn't go any channels above 11.

You should be able to tell whether you will eventually get your rebate by looking at rebatestatus.com. Once the status says you'll get it "soon", it'll take about another month.
