I don't think I've ever had a discrepancy vs. my reservation, at least not in NYC.

They also don't always get it right for their LDW in places like New York where there is a higher fee for more expensive vehicles and the website doesn't indicate that (something I found out this year...only after posting in this forum).
Well, that's because it depends on the value of the car (presumably MSRP), which isn't known at the time of reservation in most cases. I think it'll bump up to the higher rate at certain reservation classes, but a Premium could conceivably fall above or below the threshold, as could certain SUV classes. I think it's noted at the time of reservation that the LDW isn't guaranteed, but it's certainly noted in the rental agreement.
