Sigh. I guess I should have passed myself.
A few weeks ago I made a spreadsheet of all my monthly emailed AA statements for the past few years including the months that encompassed the 2013 60 miles per dollar promo.

What I saw was that in fact my ultimate miles balance did reflect the miles earned from the promo. It was just that in some consecutive months the ending balance for the previous month did not match the starting balance for the following month. But eventually it all caught up. ^

Moving forward, a spreadsheet to track my AA RDMs, EQMs, and EQPs is essential.
