I think it is luck/random. My impression is that some passengers, who would be entitled to a POUG have it blocked off on certain sectors, to prevent it from being too predictable. I had one flight where I was not offered a POUG, on what looked to me to be any empty service, which for unrelated reasons I then moved the flight to the next day. This second flight got cancelled, and I asked to be put back to the first flight. This was done under a different PNR due to other people travelling with me, and lo and behold I was offered a POUG on the same service that was denied to me just a few hours earlier. Maybe a coincidence, maybe not. On the other hand, with a big list of ET flights on my rota, I've got POUGs on offer for all but one service.
