As for the 15min waiting deadline, this is indeed for the leg TO the airport. CD have to wait you at arrival as who knows what may delay you : baggage trouble, etc.
Yes, but upgrades using miles on board / op-ups are not eligible for chauffeur drive service anyway (similar to the email upgrade offers that are sent out which are not eligible), so the guy refused at DXB has nothing to do with whether you need to book chauffeur drive at DXB on arrival. You still don't need to do so if you upgraded with miles before check-in opened or bought a revenue ticket, but it may make things slightly quicker as they can pull an address pre-printed on to the little receipt. Clearly you will need to book the car if you need a pick up to the airport upon your departure from DXB.
